Right to Receive Good Faith Estimate of Services

Under federal law, you have the right to receive a “Good Faith Estimate” of expected charges for health care items and services if you are uninsured or not using health insurance to pay for your care. This estimate shows the expected cost of services in advance, so you can understand and plan for your financial responsibility.
• You may request a Good Faith Estimate before scheduling services or at the time you schedule your appointment.
• If you schedule services at least 3 business days before your appointment, we will provide the estimate no later than 1 business day after scheduling; if scheduled at least 10 business days in advance, you will receive it within 3 business days of scheduling.
• The estimate will be provided in writing and in a format you can keep and print.
• If you receive a bill that is at least $400 more than your Good Faith Estimate, you have the right to dispute the bill.
